All parents are required to register on the OUR PATIENT PORTAL to become active users. Here you can request school forms, send message anytime, request appointmnet or refills. If you have not registered on the portal, click PATIENT PORTAL LOGIN below to get started. The fastest way is to click on the link you get from your email after you have made appointment at the office from NOREPLY@PATIENTPORTAL.NET and follow the prompts. You will only receive this email after each appointment if you do not have an active account. Please check your spam to be sure you got it.

Please print the New Patient Registration Packet and bring this, along with your Insurance Cards, Driver's license and Immunization Record to your first office visit.

We have created a great website for your convenience to suit your busy schedule, we made a few updates:
 

  • We will only confirm appointments by text or emails, please click on the confirm link to confirm your appointments. 
  • For lab results , we will inform you of your results through our secure portal, and send you a message if treatment was sent in or if we need you to call us at your convenience to discuss your child's labs. As always, labs will continue to be available on the portal once it's been reviewed and addressed.
  • You can send us message and request school forms and appointments through the portal.
  • For immediate and urgent needs you could always call us during office hours or or after hours  through our web portal. We may be able to see you virtually to safe you time and trip to urgent care.
